Five years since the royal commission into youth detention released its damning findings documents obtained by the ABC reveal the Northern Territory Government has failed to adhere to one of its key monitoring programs for more than a year. The revelations come at a time when incidents of self-harm inside the Don Dale and Alice Springs youth detention facilities have surged.
